(Update) Handling procedures in the case of becoming infected with the Novel Coronavirus or becoming a high-risk contact person
１．If you are diagnosed as having been infected by the Novel Coronavirus
The Novel Coronavirus (COVID-19) is considered a “Class 1 Infectious Disease” under the School Health and Safety Act. If you are diagnosed with the Novel Coronavirus (Covid-19), please follow the instructions below to prevent the spread of infection.
Our University protects the privacy of infected individuals from the standpoint of human rights. We believe that measures against infectious diseases are to protect the lives and the health of students and related persons, and we request everyone’s cooperation in preventing the spread of infection.
To All Students:
（１）Please receive treatment at a medical institution and refrain from coming to the university until your doctor diagnoses you as having been cured.
（２）Please report the following to your Faculty’s Office by telephone or e-mail:
※ As all offices are closed between August 8th (Saturday) and August 18th (Tuesday), please send the information by e-mail.
① Student Number・Full Name
② Name of Diagnosed Illness
③ When you started feeling symptoms
④ Your current medical condition
⑤ The date of your PCR test, and the date your results were received
⑥ Instructions received from the Public Health Center（remain at home、remain at a hotel、hospitalization、period of time, etc...）
⑦ Transmission Route
⑧ Circumstances of Infection Within Family
⑨ Your actions from two days before the onset of symptoms until this time (whether or not you entered the campus)
⑩ The last date you came to the University
⑪ If you have any contact with University-related individuals (if yes, please state the individual’s Faculty Name and Full Name)
⑫ If you have traveled overseas since January 2020（If yes, please state the places you travelled to and dates of your trip）
⑬ Whether or not you have any other underlying medical issues
⑭ Your contact information (telephone number)
To All Faculty and Staff Members:

２. If you become a high-risk contact person or have been directly in contact with a high-risk contact person
Please follow the instructions below if you have been in contact with an infected person or a high-risk contact person (such as when you have been contacted by a Public Health Center, or contacted by a family member who has become either an infected person or high-risk contact person).
To All Students:
（１）Please do not come to the University until the Public Health Center states that you may end your waiting period.
（２）Please report the following to your Faculty’s Office by telephone or e-mail:

（３）Please contact your Faculty Office as soon as the Public Health Center contacts you.
（４）After your waiting period has ended, please report your progress to your Faculty Office.
To All Faculty and Staff Members:

⑤ When you had contact with the individual
⑥ Your actions since the time of contact
⑦ Your contact information (telephone number)
（３）Please contact your Faculty Dean as soon as the Public Health Center contacts you.
（４）After your waiting period has ended, please report your progress to your Faculty Dean.
